مقایسه تمرینات تعادلی سنتی و مبتنی بر واقعیت مجازی بر زمان شروع و میزان فعالیت عضلانی سالمندان در پاسخ به آشفتگی قابل انتظار

چکیده
اختلال در اصلاحات قامتی یکی از عوامل اصلی خطر سقوط در سالمندان محسوب می‌شود. با وجود اثربخشی تمرینات تعادلی سنتی، تأثیر تمرینات مبتنی بر واقعیت مجازی بر پاسخ‌های عصبی- عضلانی به روشنی مشخص نشده است. هدف این مطالعه مقایسه این دو مداخله بر زمان شروع و میزان فعالیت عضلات مچ پا و تنه در فازهای اصلاحات قامتی اولیه، پیش‌بین و جبرانی در زنان سالمند سالم در پاسخ به آشفتگی مورد انتظار بود. 36 زن سالمند سالم (80/4 ± 73/68 سال) به‌صورت تصادفی در سه گروه (واقعیت مجازی، تمرین سنتی و کنترل) قرارگرفتند. مداخلات در 18 جلسه و 40 دقیقه انجام شد. زمان شروع و میزان فعالیت عضلات درشت نی قدامی (TA)، دوقلو (MG)، راست رانی(RF)، دوسر رانی (BF)، مورب خارجی شکم (EO)، راست شکمی(RA) و راست کننده ستون فقرات(ES) در فازهای اصلاحات قامتی هنگام عبور از مانع توسط الکترومیوگرافی اندازه‌گیری شد. نتایج آزمون تحلیل کوواریانس نشان داد در متغیر زمان شروع فعالسازی، گروه‌های مداخله نسبت به گروه کنترل بهبود داشتند، اما تفاوت معناداری بین دو گروه مشاهده نشد (05/0 <.(P همچنین، میزان فعالیت TA و MG در هر سه فاز در گروه واقعیت مجازی به‌طور معناداری نسبت به دو گروه دیگر کاهش یافت (05/0 >. (P تمرینات تعادلی مبتنی بر واقعیت مجازی می‌توانند با کاهش زمان شروع و دامنه فعالیت عضلات مچ پا اصلاحات قامتی را در سالمندان بهبود دهند و به‌عنوان راهبردی مؤثر در توانبخشی تعادلی مطرح شوند.

Comparing the Effects of Virtual Reality–Based and Traditional Balance Training on Muscle Onset Time and Activity Levels in Older Adults During Predictable Perturbations

Traditional and virtual reality (VR)-based balance training can enhance neuromuscular responses and postural adjustments. This study aimed to compare the effects of traditional versus VR-based balance training on muscle activation and anticipatory and initial postural adjustments in healthy older women in response to anticipated perturbations. In this study, 45 healthy older women (60–70 years) were randomly assigned to three groups: VR, traditional training, and control. Intervention groups performed 24 sessions training. Muscle activity of the tibialis anterior (TA), medial gastrocnemius (MG), rectus femoris (RF), biceps femoris (BF), external oblique (EO), rectus abdominis (RA), and erector spinae longissimus (ESL) was measured using surface electromyography during initial, anticipatory, and compensatory postural adjustments while crossing an obstacle. Data were analyzed using analysis of covariance (ANCOVA) and Bonferroni post-hoc tests. In the anticipatory and initial postural adjustment phases, the activity of the TA (P = 0.044 and P = 0.023, respectively) and MG (P = 0.049 and P < 0.001, respectively) muscles was significantly lower in the VR group compared to the traditional training group. No significant changes were observed in other proximal lower-limb muscles (RF, BF, EO, RA, ESL) (P > 0.05). VR-based balance training may optimize anticipatory and initial ankle muscle responses and improve postural control in older adults. Future training programs should incorporate targeted exercises to activate proximal lower-limb muscles and simulate real-life motor demands. Combining traditional and VR-based training with targeted exercise design could serve as an effective strategy for balance rehabilitation in older adults and fall risk reduction.
